WORKS

Name and summary of the specialization thesis:

"Two findings of portal hypertension: correlation evaluation of serum acid albumin gradient and esophageal varices"
Summary: In determining the etiology of ascitic fluid, it has been shown in many studies conducted in recent years that the diagnostic value of classification as exudate or transudate according to the total protein level cannot exceed 55.6%. Another clinical disadvantage of this type of classification is that it is significantly affected by infection, diuretic use and paracentesis. Contrary to the traditional classification, it has been shown in many studies that the classification of acid fluid as portal hypertension related or unrelated to portal hypertension according to the serum-acid albumin gradient (SAAG) has an accuracy rate of up to 97% in adults, although it is relatively low in children in determining the etiological cause. It has also been shown that the classification made in this way is not affected by the presence of infection, diuretic use and paracentesis.

Most studies published in the literature include ascites patients with alcoholic cirrhosis. In this study, we evaluated the correlation between SAAG and esophageal varices, which are signs of portal hypertension, in patients with non-alcoholic cirrhosis. SAAG was above 1.1 in all our patients and was compatible with the 97% accuracy rate in studies published in the literature. No correlation was found between SAAG and the degree of esophageal varices. There was only a weak correlation between the degree of varicose veins and ascites albumin (p=0.01). It is noteworthy that all patients with SAAG have esophageal varices. The lack of sufficient studies in the literature on this subject and the small number of patients make it difficult to make further comments.

Although there is a correlation between LAAG and esophageal varices in a small number of non-alcoholic cirrhosis patients in the literature, our findings support the data in the literature. Although this correlation can be demonstrated in cases with alcoholic cirrhosis, there is no correlation in cases with non-alcoholic cirrhosis.

"Effects of gingko biloba, vitamin E and melatonin on bacterial translocation in thioacetamide-induced fulminant hepatic failure in rats"
Summary: Purpose:Bacterial translocation (BT) has been established in the development of infectious complications in many serious clinical conditions such as fulminant liver failure (FHF). We aimed to investigate the effects of Gingko biloba (GB), vitamin E (Vit E) and melatonin on intestinal oxidative damage and bacterial translocation in thioacetamide (TAA)-induced PHF in rats. Materials and Methods: A total of 42 rats were divided into 5 groups. Group 1 (n=8) was the control group. Group 2 (n=10) was the TAA group, and the rats in this group received 350 mg/kg/day TAA intraperitoneally (ip) for 3 days. Oral 100 mg/kg/day GB to group 3 (n=8), orally 200 mg/kg/day Vit E to group 4 (n=8), 3 mg/day ip to group 5 (n=8). kg/day melatonin treatment was started 48 hours before the first TAA dose and continued for the next 5 days. Results: Severe hepatic and intestinal oxidative damage, increased Escherichia coli numbers in ileal aspirates, and higher CT frequencies were observed in the TAA group compared to the control group (all p's <0.001). Only GB treatment reduced hepatic oxidative damage (p<0.001). There was no difference between TAA and other antioxidant treatment groups in intestinal oxidative damage, E. Coli counts in ileal aspirates, and CT frequencies (p>0.05). Conclusion: Our results suggest that intestinal oxidative damage plays a role in the development of BT by disrupting the barrier function of the intestinal mucosa.
